Job Summary:

Responsibilities:
Provide high-quality, patient-centered care to our residents, including administering medications, monitoring vital signs, and documenting patient information
Develop and implement individualized care plans in collaboration with our interdisciplinary team
Work closely with our nursing staff to ensure seamless communication and coordination of care
Participate in quality improvement initiatives to enhance the overall care experience
