What You'll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management within senior living communities. Conduct routine rounding visits and follow-up care across assigned communities. Manage medications, diagnostics, treatment plans, and clinical interventions. Collaborate with physicians, nurses, therapists, caregivers, and community staff. Identify risks early and implement strategies that help prevent complications and hospitalizations. Support continuity of care while improving healing outcomes and quality of life. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license. Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings preferred. Strong clinical judgment and confidence managing medically complex patients. Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
