Patient Care Coordinator

Location: Optum – 30 W 24th Street, New York, NY
Pay: $19.00 per hour
Schedule:
Monday–Thursday: 8:30 AM–5:00 PM
Friday: 8:00 AM–4:00 PM

Position Overview

We are seeking a dependable, professional, and patient-focused Patient Care Coordinator to support a fast-paced healthcare environment. This role is responsible for completing a variety of administrative and patient-facing tasks that contribute to the overall success of the organization and help ensure a positive patient experience.

The Patient Care Coordinator will support daily office operations, coordinate patient services, maintain accurate information, and work closely with clinical and administrative team members.

Key Responsibilities

• Obtain and update patient demographic and insurance information.
• Verify insurance coverage on the date of service.
• Obtain patient signatures for required forms and documents.
• Check patients in and out for appointments.
• Answer inbound calls and make outbound calls as needed.
• Schedule, reschedule, confirm, and coordinate patient appointments.
• Process referrals and complete follow-up scheduling with specialists.
• Maintain work queues, inboxes, and pending patient actions.
• File, fax, scan, and maintain patient medical records.
• Monitor fax queues and assign documents to the appropriate electronic patient chart.
• Collect and process patient information in accordance with payment security guidelines.
• Coordinate patient care through scheduling and tracking systems.
• Communicate professionally with patients, families, providers, and other healthcare professionals.
• Maintain compliance with HIPAA, OSHA, and company policies and procedures.
• Work collaboratively with the care team in a fast-paced environment.
• Cross-train and provide coverage at other locations as needed.
• Perform additional administrative and clerical duties as assigned.

Team Environment

The Patient Care Coordinator will work with a multidisciplinary team of approximately 21 team members , including providers, Medical Assistants, Registered Nurses, Licensed Practical Nurses, Patient Service Associates, and Echo Technicians.

The team works in a fast-paced and collaborative environment where strong interpersonal skills, professionalism, teamwork, and reliability are essential.

Required Qualifications

• Minimum of 2 years of healthcare experience
• Strong communication and customer service skills
• Ability to work efficiently in a fast-paced medical office
• Professional, mature, dependable, and responsible
• Strong organizational skills and attention to detail
• Ability to work effectively as part of a multidisciplinary team
• Ability to maintain patient confidentiality and follow HIPAA requirements

• Stable work history and demonstrated longevity in previous positions
• Experience supporting patient scheduling, registration, referrals, or medical office operations

License or Certification

No professional license or certification is required.
