Key Responsibilities\n

• Conduct ongoing evaluations of patients’ needs, regularly adjusting care plans in partnership with patients and their caregivers to reflect any changes in their condition.

• Develop individualized care plans based on thorough assessments, establishing clear goals for positive health outcomes in collaboration with physicians and interdisciplinary teams.

• Implement therapeutic interventions, including crisis counseling and psychotherapy for individuals and families, adapting techniques to meet diverse needs.

Patient Population / Client Focus\n

You will work primarily with hospice patients who face various acute mental health diagnoses, including grief, depression, anxiety, and substance use disorders. Your ability to create rapport and trust with both patients and their families will be instrumental in providing effective support. Qualifications\n

Compensation and Benefits\n

The salary range for this position is competitive, with annual compensation varying between USD 80,000 and 122,000, depending on experience and qualifications.
