Setting: Outpatient department within a hospital (home health focus with outpatient emphasis)

This travel - temporary assignment is for a PT who will work in an outpatient department within a hospital setting, with primary emphasis on outpatient therapy and a smaller component of LTC/Skilled Swing tasks. The assignment runs for 13 weeks with a strong possibility for extension. Housing is not included; a list of housing options can be provided upon request.

Responsibilities
- Deliver individualized physical therapy services in an outpatient hospital setting, including assessment, treatment planning, implementation, and progress monitoring
- Develop and implement customized treatment plans to maximize functional outcomes and return to activity
- Educate patients and families on exercise programs, self-management, and safety to support ongoing progress
- Reassess patient progress regularly and adjust plans to meet functional goals
- Document evaluations, treatment plans, progress notes, and outcomes clearly in the medical record
- Collaborate with interdisciplinary team members to coordinate care and ensure consistent communication

Requirements
- Availability to work a 40-hour week, Monday through Friday, 8:00 am–5:00 pm
- Ability to perform PT duties in an outpatient hospital setting with occasional LTC/Skilled Swing tasks
- Strong communication and documentation skills
- Comfort working within a collaborative multidisciplinary team
- Eligibility to participate in a 13-week travel assignment and to travel to Baker, MT for the duration of the contract
