Auberle's mission is to help build strong individuals, families, and communities.In 2019, Ward Home joined the Auberle family and has three programs in our Young Adult Services Department.**Program Description:**The Family Healing Center (FHC) is an innovative licensed inpatient 3.5 drug and alcohol facility dedicated to comprehensive addiction treatment and family healing. FHC provides a holistic approach to recovery, offering personalized care for individuals struggling with addiction while actively involving their families in the healing process.Through a combination of evidence-based therapies, trauma-informed care, and family support programs, FHC addresses the underlying causes of addiction, promoting emotional and behavioral well-being. Children live on-site with their parent during treatment, allowing families to remain connected and supported throughout the recovery journey. The program empowers individuals to rebuild their lives and strengthens family bonds, fostering a supportive environment for long-term recovery and resilience.**Position Title and Overview:**Case Manager / Social Worker* Assist clients and families with enrollment in benefits such as SNAP, WIC, Medicare, housing assistance, and other needs identified during intake.* Accompany clients to court hearings, including legal and Children, Youth, and Family (CYF) proceedings, ensuring they are supported and informed.* Facilitate access to community resources such as food banks, diaper banks, and other essential services.* Maintain accurate documentation of all client and family activities in the Electronic Health Record and KIDS systems for continuity of care and compliance.**Location and Travel Requirements:**This position is primarily located in Mt. Oliver.
