Job Summary The Medical Social Worker LCSW provides clinical counseling and consulting services to patients, families, and staff members and assists in facilitating the development of new and expanded social work services and programs within the hospital and planning for discharge and aftercare. The role focuses on providing patient-centered care in a collaborative, team-oriented environment. Responsibilities Interview patients and family members to obtain relevant biopsychosocial information and determine the effect of illness and trauma on the patient and family. Identify complex, multiple, social, emotional, financial, and environmental factors that contribute to or interfere with the patient’s ability to benefit from medical care. Develop and implement a social work treatment plan for patients and families based on an analysis of psychosocial factors and the medical illness, including discharge planning and insurance utilization review on appropriate units. Utilize appropriate treatment modalities, including individual, family, and group interventions. Provide counseling on family problems, coping with illness, trauma, loss of function, and grief. Participate as part of the multidisciplinary team in case and family conferences to assist in formulating overall patient care plans and interpret psychosocial aspects of the patient’s illness to other staff. Provide community and professional staff training regarding patient psychosocial needs or coping with illness, grief, and loss. Provide evaluation, support, follow-up referrals, planning, and reports as needed for victims of child abuse, rape, elder abuse, and family violence. Provide evaluation and referral services for patients who are alcohol or drug dependent. Qualifications LCSW license required. Master of Social Work (MSW) required.
