Key Responsibilities

• Conduct in-depth psychosocial assessments to inform patient treatment plans and discharge strategies, working closely with the healthcare team.
• Maintain accurate documentation of clinical interactions, assessments, and treatment progress in compliance with department policies.
• Provide one-on-one counseling and crisis intervention to patients facing emotional and behavioral challenges, ensuring immediate support and long-term care strategies.
• Consult with families and significant support systems to coordinate treatment plans and effective discharge planning, aiming to empower patients and navigate complex emotional landscapes.
• Utilize electronic medical records and other software systems to manage patient information efficiently and securely, collaborating in a timely manner with the multidisciplinary team.
• Stay current with best practices in social work and hospice care to continually enhance the quality of services provided.

Patient Population / Client Focus

In this role, you will primarily work with patients and families facing terminal illnesses and transitions in end-of-life care. Your focus will be on addressing the complex emotional, social, and practical challenges they encounter. This requires a deep understanding of grief, loss, and the holistic needs of both the patient and their support systems. Your interventions will contribute significantly to improving their quality of life and emotional healing during this critical time.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Active licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
• At least 2-3 years of clinical social work experience, specifically in a healthcare or hospice setting.
• Proficiency in using electronic health record systems and various software platforms for documentation and communication purposes.
• Adept at managing multiple priorities in a fast-paced environment, demonstrating flexibility and efficiency.

Preferred Experience

Compensation and Benefits
