Key Responsibilities

• Conduct comprehensive virtual psychosocial assessments to gain insights into clients' mental health needs and strengths.
• Provide high-quality evidence-based therapy and counseling services to individuals, families, and groups, utilizing therapeutic modalities that best fit client profiles.
• Collaborate seamlessly with interdisciplinary teams, including psychiatrists, nurses, and other social workers, to coordinate integrated care that aligns with clients’ wellness goals.
• Advocate vigorously for clients’ rights, ensuring they have access to essential community resources and support services, tailored to their specific needs.
• Document all client interactions, assess treatment progress, and evaluate outcomes in electronic health records with meticulous attention to detail.
• Engage actively in team meetings, case conferences, and continuing education opportunities to reinforce clinical skills and stay abreast of best practices in social work.

Patient Population / Client Focus

Your primary focus will be on clients within the hospice care community, addressing their unique mental health challenges as they navigate their end-of-life journey. You will provide empathetic support to individuals facing significant life transitions and their families, facilitating emotional healing and resilience. This role requires a keen understanding of end-of-life issues, grief, and the psychosocial aspects of terminal illness to effectively support your clientele.

Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
• A minimum of two years of post-licensure experience providing clinical social work services, preferably in a mental health or hospice setting.
• Expertise in conducting diagnostic assessments and developing effective treatment plans that are adhered to through therapy sessions.
• Exceptional interpersonal and communication skills, enabling you to establish rapport and trust with a diverse population.
• A strong commitment to cultural competence, social justice, and ethical practice principles, ensuring respectful and equitable care for all clients.

Preferred Experience

Experience working with hospice or palliative care populations is highly desirable. Familiarity with evidence-based practices such as Dialectical Behavior Therapy (DBT), Acceptance and Commitment Therapy (ACT), and other relevant therapeutic modalities will be an advantage. Your ability to adjust therapeutic interventions based on individual client needs will further enrich our clients’ experiences.
