Work Environment

Key Responsibilities

• Collaborate with multidisciplinary healthcare teams to develop and implement comprehensive treatment and discharge plans based on thorough psychosocial assessments.
• Conduct timely assessments of patients’ emotional, behavioral, and social needs, and provide evidence-based interventions accordingly.
• Maintain comprehensive clinical records and progress notes, ensuring that all documentation complies with departmental policies and regulatory standards.
• Engage in ongoing counseling and crisis intervention strategies, including facilitating support for patients experiencing significant emotional distress.
• Effectively communicate and coordinate with patients' families and support systems to create robust treatment plans that accommodate the unique needs of the client.

Patient Population / Client Focus

This role specifically addresses the needs of patients within the hospice setting, who often face complex emotional challenges during their end-of-life journey. You will work closely with individuals and families navigating grief, anxiety, and various psychosocial issues, ensuring they receive compassionate, tailored support. Qualifications

• A Master’s degree in Social Work or Counseling from an accredited institution.
• Active licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
• At least 2-3 years of relevant clinical experience in a social work or mental health setting.
• Proficient computer skills, including experience with electronic medical records, word processing, and spreadsheets.
• Strong ability to prioritize tasks and work efficiently in a dynamic and fast-paced environment.
