Key Responsibilities

• Conduct thorough clinical assessments to understand clients' needs, challenges, and goals, with an emphasis on holistic treatment strategies.
• Create and implement individualized treatment plans that incorporate evidence-based practices and solution-based interventions.
• Deliver a variety of therapeutic modalities including individual counseling, family therapy, and group sessions designed to empower clients and facilitate coping strategies.
• Maintain accurate documentation of client interactions and treatment progress in compliance with state regulations and organizational policies.
• Collaborate with a multidisciplinary healthcare team, including doctors, nurses, and spiritual care providers, to ensure a seamless continuum of care.
• Engage in regular case consultations and peer reviews to reflect on and enhance clinical practices, ensuring the highest quality of care.
• Provide education and support to clients and their families regarding mental health issues, care options, and available community resources.
• Continuously update your knowledge of the latest research, tools, and techniques in social work to improve patient care outcomes.

Patient Population / Client Focus

This role will primarily focus on providing support to terminally ill patients and their families within a hospice setting. The clientele varies in age and background, often facing complex emotional, social, and logistical issues surrounding end-of-life care. Your expertise will help in navigating these challenges, providing comfort and clarity in a time of uncertainty.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Current and valid LICSW license in the state of Washington.
• Minimum of 2 years of clinical experience with a focus on mental health or hospice-related services.
• Demonstrated proficiency in solution-based psychotherapy interventions tailored to individuals and families.
• Strong communication skills and ability to build rapport with a diverse range of clients.
• Ability to work dynamically within a team environment, contributing to collaborative treatment plans.

Preferred Experience

• Experience working within a hospice or end-of-life care setting would be a significant advantage.
• Familiarity with electronic health records (EHR) systems and online documentation processes.
• Previous involvement in community outreach programs focused on mental health awareness.

Compensation and Benefits
