Clinical Social Worker - Remote Hospice Care Specialist

This full-time position offers the unique opportunity to provide remote support to clients receiving hospice care in Trenton, New Jersey. Work Environment

This position is remote, allowing you the flexibility to manage your time while working with a compassionate team in the comfort of your home. Though primarily virtual, you’ll collaborate seamlessly with hospice caregivers, nurses, and physicians. Your schedule will be Monday through Friday from 9 AM to 5 PM, providing a consistent work-life balance.

Key Responsibilities

• Work collaboratively with interdisciplinary teams to develop and implement individualized care plans for hospice clients, assessing their psychological and social needs.
• Conduct comprehensive case reviews, ensuring all aspects of care are considered and that resources align with the clients' and families’ needs.
• Provide psychoeducation to clients and their families, delivering crucial information regarding mental health resources and supportive intervention strategies.
• Utilize evidence-based practices in assessments and treatment design, ensuring adherence to the latest advancements in mental health care.
• Document all client interactions in a professional and timely manner using our integrated electronic health record (EHR) system.
• Advocate for clients’ needs, coordinating with healthcare providers, community resources, and family members to ensure comprehensive support.

Patient Population / Client Focus

Your work will predominantly focus on clients receiving hospice care, many of whom may be dealing with terminal illness, diverse psychosocial challenges, and grief. You will be responsible for addressing the complex emotional dynamics experienced by clients and their families, providing sensitive support through times of profound change.

Qualifications

• A Master’s degree in Social Work (MSW) from an accredited institution.
• An active Licensed Clinical Social Worker (LCSW) license in the state of New Jersey.
• Proficiency in clinical evaluations, treatment planning, and knowledge of hospice care standards and procedures.
• Strong interpersonal skills, with an ability to foster relationships and collaborate effectively in a team setting.

Preferred Experience

• At least 2 years of clinical social work experience, particularly in hospice, palliative care, or similar health settings.
• Familiarity with the challenges faced by patients with chronic or terminal illnesses and their families.
• Experience with electronic health records (EHR) management and documentation.

Compensation and Benefits
