Key Responsibilities

• Conduct comprehensive evaluations and assessments of clients, ensuring that each individual receives personalized and holistic care.
• Engage in the development and implementation of Individualized Family Service Plans (IFSPs) to guide therapeutic interventions and monitor patient outcomes.
• Collaborate with family members and allied health professionals to enhance understanding and support concerning the emotional and practical needs of individuals receiving hospice care.
• Utilize evidence-based practices and the Natural Learning Environment methodology to create joint plans with families that align with their priorities and concerns.
• Maintain compliance with all federal and state regulations governing social work practices while ensuring the health and safety of clients and staff.
• Provide ongoing support to families, equipping them with information, skills, and strategies to promote the emotional and developmental needs of their loved ones.
• Implement tailored interventions in a compassionate manner that respects the wishes and dignity of both patients and their families.

Patient Population / Client Focus

This role primarily focuses on individuals within the hospice care setting, including children and families who are experiencing significant life transitions due to illness. The LCSW will address both the psychosocial and emotional challenges faced by these clients, ensuring they feel valued, understood, and supported throughout their journey. Qualifications

• Master's Degree or Doctorate in Social Work (MSW or DSW) required.
• Licensed Clinical Social Worker (LCSW) certification is mandatory.
