Position Overview

The American Counseling Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team. Key Responsibilities

• Conduct comprehensive psychosocial assessments by performing interviews, observations, and thorough reviews of medical records, identifying emotional, social, and environmental factors affecting patients' recovery and well-being.
• Deliver targeted counseling and emotional support to both patients and their families, addressing issues of adjustment to diagnosis, emotional distress, and end-of-life decisions.
• Work closely with the rehabilitation and healthcare team to develop, implement, and evaluate individualized treatment plans that meet patients' psychosocial needs and align with their overall medical care.
• Guide patients in navigating community resources, such as support groups, vocational rehabilitation programs, or housing assistance, helping them to access the services they require.
• Educate patients and their families regarding the available community resources and the options for care post-rehabilitation, providing the necessary support for their transition.
• Participate actively in discharge planning and care coordination meetings to ensure continuity of care and support patient reintegration into their homes and communities.
• Maintain accurate and timely documentation of patient interactions, treatment progress, and resource access to comply with regulatory standards and optimize care quality.

Patient Population / Client Focus

Our hospice center serves a diverse group of patients experiencing life-limiting illnesses, including those with chronic conditions, neurological disorders, and terminal diagnoses. As an LCSW, your role will center around understanding and addressing the complex emotional landscapes these individuals navigate, while also providing support to their families, who are integral to the healing process.

Qualifications

• A valid Licensed Clinical Social Worker (LCSW) credential.
• A Bachelor's Degree in Social Work or a closely related field; a Master's degree is preferred.
• A minimum of one year of related experience in the healthcare or rehabilitation sector, preferably working with terminally ill patients.

Preferred Experience

Ideal candidates will have experience in a hospice or palliative care setting, demonstrating an understanding of the unique challenges faced by patients and families during end-of-life care. Familiarity with community resources in Riverside, California, would be an asset.
