Key Responsibilities

• Meet with clients promptly during scheduled appointments to provide timely interventions and support.
• Conduct thorough clinical assessments to develop individualized treatment plans that reflect each client’s unique needs, emphasizing follow-up on their progress.
• Actively participate in multi-disciplinary team meetings, collaborating with healthcare professionals to construct and refine holistic treatment strategies.
• Continuously monitor and evaluate client situations, conducting follow-ups that ensure ongoing improvements and adjustments in care plans.
• Partner with educators and family members to address behavioral and emotional challenges faced by students, particularly in the hospice context.

Patient Population / Client Focus

You will primarily work with patients in hospice care, addressing not only their emotional and psychological needs but also those of their families. Qualifications

• A Master’s Degree in Social Work from an accredited institution.
• An active and unrestricted Clinical Social Worker License (LCSW).
• A minimum of 2 years of professional experience in a behavioral health or social service setting, with a preference for hospice or palliative care experience.
• Demonstrated skills in conflict resolution and crisis intervention.
• Effective competency in taking psychosocial histories and conducting comprehensive case management evaluations.

Compensation and Benefits

This position offers a competitive annual salary ranging from $90,000 to $100,000, commensurate with experience and qualifications.
