Key Responsibilities

• Conduct thorough psychosocial assessments to identify clients' mental health needs, strengths, and available resources.
• Provide individualized, family, and group therapy utilizing evidence-based practices such as cognitive-behavioral and trauma-focused therapies.
• Actively collaborate with interdisciplinary teams, including psychiatrists and psychologists, to establish cohesive and holistic treatment plans.
• Facilitate crisis intervention and develop risk assessment strategies to enhance client safety and well-being.
• Maintain accurate documentation in compliance with legal and ethical standards to ensure continuity of care.
• Lead group therapy sessions and psychoeducation classes aimed at strengthening community support and understanding for clients and their families.
• Work directly with patients at our affiliated hospice center, providing tailored therapy to address end-of-life issues and grief.

Patient Population / Client Focus

In this role, you will primarily support individuals and families coping with terminal illnesses as well as those navigating the emotional complexities of hospice care. Your expertise will help patients address anxiety, depression, and grief while reinforcing their quality of life during critical transition periods. You will utilize empathy and cultural competence to engage with a diverse clientele, ensuring that each person feels respected and valued.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a mental health or outpatient setting, ideally with a focus on hospice or palliative care.
• Proficient in evidence-based therapeutic modalities with a solid grasp of mental health diagnosis and treatment strategies.
• Exceptional interpersonal and communication skills, with a strong ability to assess and respond to client needs effectively.

Preferred Experience

• Experience working as part of a multidisciplinary team within the hospice care setting.
