Work Environment

This position is entirely remote, allowing for flexibility while serving clients from the comfort of their own homes. As part of our hospice team, you will work collaboratively with an interdisciplinary group of healthcare professionals, including doctors, nurses, and psychologists, to ensure that all aspects of patient care—including psychological, emotional, and social needs—are addressed holistically.

Key Responsibilities

• Conduct in-depth psychosocial assessments to identify clients' mental health concerns, strengths, and available support systems.
• Deliver high-quality individual, family, and group therapy by employing a variety of evidence-based therapeutic modalities, such as cognitive-behavioral therapy and trauma-focused therapy.
• Create personalized treatment plans tailored to each client’s unique needs, goals, and circumstances, adjusting them as necessary based on progress and feedback.
• Work closely with psychiatrists and other mental health professionals to develop and implement coordinated and effective client-centered care strategies.
• Provide crisis intervention and risk assessments, ensuring client safety and well-being during high-stress situations.
• Maintain precise and timely documentation of client interactions, assessments, and treatment updates in compliance with ethical and legal standards.
• Facilitate therapeutic group sessions, psychoeducation classes, and support groups to build community and provide peer support among clients.

Patient Population / Client Focus

This role focuses primarily on clients receiving hospice care, many of whom may be dealing with complex emotional issues as they process grief, loss, and life transitions. You will engage with diverse individuals and families, providing compassionate support and guidance tailored to their specific emotional and psychological needs.

Qualifications

• A Master’s degree in Social Work from an accredited institution, ensuring a solid foundation in clinical knowledge and practice.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) to practice independently and provide therapeutic services.
• At least two years of experience working in a mental health or outpatient setting, demonstrating a commitment to best practices in social work.
• Strong proficiency in various evidence-based therapeutic techniques, as well as excellent interpersonal, communication, and assessment abilities.

Preferred Experience

• Experience in hospice or palliative care settings, particularly in working with terminally ill patients and their families.
• Familiarity with interdisciplinary teamwork in a healthcare setting, emphasizing holistic approaches to patient care.
• Advanced understanding of specific challenges related to grief, loss, and end-of-life issues.

Compensation and Benefits
