Work Environment

This position is primarily remote, allowing you to manage and support patients creatively and efficiently from your home or designated offsite office. Despite the primarily virtual nature of the role, you will maintain close collaboration with interdisciplinary teams in various healthcare settings, ensuring coordination of care that meets the unique needs of patients and their families. Key Responsibilities

• Independently manage a caseload of patients, performing comprehensive assessments to identify social, emotional, financial, and cultural factors that influence their treatment plans.
• Utilize various therapeutic modalities to facilitate timely transitions through the healthcare continuum, advocating for patients' rights and self-determination.
• Lead multidisciplinary care coordination efforts by addressing protective service issues, advance directives, and complex psychosocial challenges affecting high-risk patients.
• Develop and implement treatment plans and crisis intervention strategies, ensuring prompt evaluations and responses to the needs of patients and families.
• Educate and coordinate resources for patients, families, and caregivers, ensuring adequate support systems are in place to mitigate readmission risks.
• Collaborate with case managers and healthcare teams to integrate psychosocial assessments into comprehensive treatment and discharge plans.
• Supervise and mentor social work staff, including MSW interns, while providing leadership in evolving practice standards for optimal service delivery.

Patient Population / Client Focus

The LCSW will primarily serve hospice patients and their families, focusing on end-of-life care needs. Qualifications

• Must possess a valid LCSW license in California.
