Work Environment

This position allows for a flexible remote work setting while ensuring that our clients receive the highest standard of care. Flexibility in scheduling is essential, as client visits may require evening or weekend availability depending on their requirements.

Key Responsibilities

• Conduct in-depth psychosocial assessments for seniors to evaluate their emotional, mental, and social needs.
• Collaborate with clients and their families to develop tailored care plans that reflect their unique goals and preferences.
• Provide ongoing counseling and emotional support, assisting seniors in navigating life transitions, such as bereavement or major health changes.
• Advocate for clients' rights, ensuring they have access to essential community resources and support services.
• Work collaboratively with an interdisciplinary team to enhance overall client care and well-being.
• Perform regular client visits to assess their living conditions, monitor health status, and address any safety concerns.
• Facilitate effective communication and conflict resolution among family members regarding the care of senior clients.
• Educate clients and their families on community resources, support groups, and programs that can improve their social engagement.
• Maintain comprehensive and accurate documentation, including client assessments, care plans, and progress notes, ensuring compliance with ethical standards.
• Stay updated on current social work practices, policies, and regulations to provide informed care.

Patient Population / Client Focus

This role focuses particularly on working with seniors, a demographic often facing distinct challenges related to aging. The successful candidate will support clients and their families in managing feelings of anxiety, depression, or loneliness as they navigate their later stages of life.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Clinical Social Worker (LCSW) is required.
• At least two years of experience in a social work role, preferably within a community center or a geriatric healthcare environment.
• Comprehensive understanding of aging-related issues and local community resources available for seniors.
• Exceptional assessment, counseling, and problem-solving abilities, paired with a high level of empathy and compassion for clients.
• Excellent communication and interpersonal skills are vital for building rapport and working effectively with interdisciplinary teams.
• Competence in using electronic health record (EHR) systems and related software.

Preferred Experience

Compensation and Benefits
