Key Responsibilities

• Conduct in-depth psychosocial assessments of seniors, evaluating emotional, mental, and social factors affecting their health and well-being.
• Develop personalized care plans in collaboration with clients and their families, tailoring interventions to meet specific goals and preferences.
• Provide one-on-one counseling and emotional support, equipping clients with strategies to cope with life changes, including health issues, bereavement, and transitions in living situations.
• Advocate for clients’ rights and facilitate access to community resources, ensuring they receive comprehensive support and services.
• Collaborate with an interdisciplinary team to coordinate care, handle complex cases, and share insights to enhance service delivery.
• Conduct regular follow-up visits to monitor clients’ well-being and adjust care plans as necessary, addressing any new developments or safety concerns.
• Educate clients and families about available support systems, community resources, and social services that can enhance overall quality of life.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits
