Position Overview

The Haven Family Therapy is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our remote team in Port Saint Lucie, Florida. Work Environment

This position is primarily remote, allowing for flexible work arrangements while maintaining direct engagement with clients. You will be supported by a robust team of healthcare professionals dedicated to interdisciplinary collaboration. Standard work hours are Monday to Friday, from 8:00 AM to 4:30 PM, although occasional evenings or weekend hours may be required depending on client needs. The ability to be on-call for urgent situations is essential to ensure comprehensive care.

Key Responsibilities

• Conduct comprehensive assessments to determine the psychosocial needs of clients, considering their emotional, mental, and social circumstances.
• Collaborate with clients and their families to create individualized care plans that reflect their specific goals and values.
• Provide counseling services, assisting clients in navigating life transitions, loss, and other challenges while actively promoting emotional health.
• Act as an advocate for clients, ensuring they have access to necessary community resources and services that enhance their daily living.
• Foster partnerships with interdisciplinary teams, actively participating in care coordination with healthcare providers, nurses, and other social services.
• Engage in regular follow-up visits to evaluate clients' living conditions, monitor well-being, and address any emerging concerns promptly.
• Facilitate open communication among family members involved in the care of client seniors, helping to mediate conflicts and promote understanding.
• Educate clients and families about community resources, support groups, and programs that can provide additional connections and enhance their quality of life.
• Maintain meticulous records of client assessments, care plans, and progress notes, ensuring thorough documentation compliant with legal and ethical standards.
• Stay abreast of current practices in social work, attending professional development opportunities to enhance personal and professional growth.

Patient Population / Client Focus

The primary focus of this role is on the senior population within the community. You will work closely with elderly clients and their families, addressing challenges related to aging, mental health, and social support. A strong understanding of aging-related issues and compassion for the complexities faced by older adults is crucial.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Must hold a current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of professional social work experience is essential, preferably in community health or geriatric settings.
• Strong knowledge of social work principles, the aging process, and available community resources is necessary for effective care.
• Exceptional assessment, counseling, and interpersonal skills are crucial for effective client interaction.
• Ability to empathize and connect compassionately with seniors and their families is essential for fostering trust.

Compensation and Benefits

The salary for this position ranges from $60,000 to $100,000 yearly, commensurate with experience and qualifications.
