Savant Care Inc. is looking for a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in providing essential mental health support to individuals and families within our Hospice Center in Corona, California. Work Environment

You will be supported by an administrative team focused on reducing your administrative burden, allowing you to concentrate on delivering high-quality patient care.

Key Responsibilities

• Conduct thorough initial assessments for patients receiving hospice care, evaluating their mental health needs and conditions through comprehensive interviews and validated tools.
• Develop individualized treatment plans that integrate the patients' unique circumstances, preferences, and clinical diagnosis, ensuring alignment with care goals.
• Provide therapy sessions in various formats—individual, couples, family, and group settings—addressing the psychological needs of both the patients and their families in coping with advanced illness.
• Monitor patient progress regularly, using established metrics to assess improvement and adjust treatment plans as needed to meet evolving needs effectively.
• Offer crisis intervention services and referrals to community resources or emergency services when patients face acute psychological distress.
• Maintain meticulous and up-to-date clinical documentation in compliance with state and federal regulations, ensuring the confidentiality and rights of our patients.
• Collaborate with an interdisciplinary team of healthcare professionals, including physicians, nurses, and chaplains, to ensure patients receive comprehensive care that addresses all aspects of their well-being.
• Participate in regular staff meetings and engage in ongoing training opportunities to develop skills and stay current with best practices in clinical social work.

Patient Population / Client Focus

The primary focus of our work is on patients in hospice care, which includes individuals facing serious, life-limiting illnesses. You will work with patients and families to provide emotional support, address mental health concerns, and enhance the quality of life during a critical time. Sensitivity to the emotional and psychological needs of these patients is paramount, and successfully integrating palliative care principles into your practice will be essential.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and unrestricted California LCSW license is required.
• Active license to practice independently as a mental health therapist.
• Minimum of 2 years of professional experience providing mental health services, preferably within hospice or end-of-life care settings.
• Demonstrated exceptional communication and interpersonal skills, including the ability to relate to diverse populations in sensitive situations.
• A proactive approach to problem-solving, with the ability to work independently and effectively as part of a multi-disciplinary team.

Preferred Experience

• Experience with electronic health record systems is highly preferred to aid in efficient documentation and patient management.
• Background in crisis intervention and assessment techniques applicable to end-of-life scenarios.
• Familiarity with community resources and support services that can benefit patients and families navigating hospice care.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and expertise. In addition to the salary, we provide a comprehensive benefits package that includes:
