Key Responsibilities

• Conduct comprehensive mental health assessments for patients and their families regarding emotional and psychological needs as they navigate the end-of-life process.
• Develop and implement individualized treatment plans tailored to address each patient's unique circumstances, focusing on emotional support, trauma processing, and psychoeducation.
• Provide a range of evidence-based therapeutic interventions, including grief counseling, cognitive-behavioral therapy (CBT), and support for managing anxiety and depression related to terminal illness.
• Facilitate individual therapy sessions and support groups to foster emotional healing, resilience, and coping mechanisms in patients and family members.
• Coordinate with a multidisciplinary team of healthcare professionals to develop holistic strategies for patient care and provide family counseling services.
• Offer immediate crisis intervention when needed to ensure patient safety and emotional stability, utilizing established de-escalation techniques.
• Empower patients and caregivers through psychoeducation on mental health issues, providing resources and strategies to navigate their journey.
• Maintain thorough and precise documentation of patient interactions, treatment plans, and progress notes, adhering to all legal and ethical standards.
• Participate in virtual team meetings and professional development opportunities to enhance your clinical skills and share insights.
• Monitor changes in patients’ mental health statuses and make timely referrals to specialized care as necessary.

Patient Population / Client Focus

This position specifically focuses on providing mental health support to patients within a hospice care context. You will engage with individuals experiencing various life-limiting conditions, offering tailored interventions that address complex emotional responses to grief, loss, and existential distress. Providing supportive care to family members is also a vital aspect of this role, equipping them with the tools needed to manage their own emotional health as they support their loved ones.

Qualifications

• A Master’s Degree in Social Work from an accredited institution is required.
• A current and valid LCSW licensure in the state of Texas is essential.
• At least two years of post-licensure experience, preferably within hospice, end-of-life care, or similar mental health settings.
• Demonstrated knowledge of mental health disorders, trauma-informed care, and relevant evidence-based therapeutic modalities.
• Exceptional skills in conducting thorough mental health assessments and formulating appropriate treatment strategies.
• Strong crisis management capabilities and the ability to stay level-headed in high-pressure scenarios.
• Excellent communication and interpersonal skills to effectively engage with a diverse patient population and their families.
• A commitment to ethical practice and the ability to establish trust and rapport with patients.
• Experience with electronic health record (EHR) systems and proficiency in using communication technology for virtual care delivery.

Preferred Experience

• Previous work within a hospice or palliative care setting is advantageous.
• Familiarity with grief counseling techniques and therapeutic adaptations for terminal illness.
• Experience collaborating with interdisciplinary teams in a virtual environment.

Compensation and Benefits
