Key Responsibilities

• Conduct comprehensive assessments and develop tailored treatment plans for individuals in hospice care, addressing their psychological, emotional, and social needs.
• Utilize a variety of therapeutic techniques, including cognitive-behavioral therapy, mindfulness practices, and supportive counseling, to promote patient well-being.
• Maintain minimal paperwork by leveraging our integrated platform, which automates documentation and allows you to focus on clinical interactions.
• Facilitate referrals to appropriate community resources and support programs to enhance the overall quality of life for clients and their families.
• Participate in regular team meetings and training sessions, sharing insights and strategies to improve client care and professional practices.

Patient Population / Client Focus

Your primary focus will be on patients receiving hospice care—a sensitive population that requires competent and empathetic social work services. You will work with clients experiencing grief, anxiety, depression, and related mental health concerns as they navigate end-of-life processes. Your role will be vital in helping clients and their families find comfort and coherence during a challenging time.

Qualifications

• A Master's Degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology is required.
• Current, unrestricted LCSW license in California with no disciplinary actions.
• Strong communication skills are essential for effective interactions with clients, families, and administrative personnel.
• Experience in hospice, palliative care, or end-of-life counseling is highly valued.
• Ability to work independently while managing your caseload flexibly and effectively.
• A dedicated, private space for conducting teletherapy sessions is necessary.

Preferred Experience

• Previous experience working with patients in hospice settings or with terminal illnesses.
• Familiarity with electronic health records and teletherapy platform functionality.
• Additional certifications or dual licensure in other states are considered a plus.

Compensation and Benefits

The annual salary for this role ranges from USD 80,000 to 104,000 , commensurate with experience and expertise. In addition to competitive compensation, our benefits package includes:
