In this full-time role, you will provide vital counseling services to individuals and families affected by substance abuse and mental health issues, ensuring they receive the support and resources necessary for their recovery journey. You will work within a hospice setting, focusing on patients dealing with complex, co-occurring disorders.

This position offers the flexibility of a fully remote work setting, allowing you to conduct therapy sessions and client assessments from the comfort of your own home. Key Responsibilities\n

Your role as an LCSW will involve a variety of critical tasks, including:

• Conducting thorough assessments of clients' counseling needs, including psychosocial evaluations and comprehensive history taking.
\n
• Utilizing DSM criteria to make informed substance abuse diagnoses and identifying appropriate levels of care based on ASAM standards.
\n
• Preparing and actively managing the substance abuse counseling component of treatment plans to guide clients through their recovery process.
\n
• Developing and implementing discharge and continuum of care plans that facilitate a smooth transition to necessary follow-up services.
\n
• Accurately documenting all instances of counseling provided, assessments made, and referrals in each client's clinical record to ensure continuity of care.
\n
• Maintaining an updated caseload and participating in treatment planning duties alongside ongoing clinical charting.
\n
• Coordinating support services for clients and their families, contributing to a comprehensive approach to treatment.
\n
• Engaging with external stakeholders, such as legal representatives and educational institutions, to secure essential resources for clients.
\n
• Efficiently managing crisis intervention situations by leveraging a multi-disciplinary team approach.

Patient Population / Client Focus\n

This role primarily serves clients confronted by co-occurring mental health and substance use disorders within a hospice environment. You will interact with individuals facing end-of-life issues as well as their families, providing them with emotional support and guidance throughout their treatment journey. Your expertise will be instrumental in delivering tailored therapeutic interventions that resonate with diverse client backgrounds.

Qualifications\n

To be considered for this position, candidates must possess the following:

• A valid LCSW license in the state of New York.

• A Bachelor’s degree in Social Work or a related field.

• Strong ethical standards, professionalism, and the ability to establish excellent therapeutic boundaries.

Preferred Experience\n

Candidates with a background in co-occurring counseling, substance abuse treatment modalities, and experience working with hospice patients will be given preference. Familiarity with electronic health record systems for documentation is beneficial.
