Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through direct interviews, observational methods, and thorough reviews of medical records to identify emotional, social, and environmental factors influencing patient recovery.

• Provide empathetic counseling and emotional support to patients and their families, helping them navigate adjustment issues related to illness or disability.

• Collaborate actively with the interdisciplinary rehabilitation team to develop and implement individualized treatment plans that reflect each patient's unique psychosocial needs and rehabilitation goals.

• Assist patients in accessing vital community resources, including local support groups, vocational rehabilitation services, and housing assistance programs.

• Educate and empower patients and their families regarding post-rehabilitation community resources available to ensure a smooth transition to increased independence.

• Engage in discharge planning efforts, facilitating transitions for patients moving back to home-based care or various other care settings.

Patient Population / Client Focus\n

Your key focus will be on a diverse group of patients at different stages of their rehabilitation journey. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
