Start Date: ASAP | Salary Range: $140,000 - $150,000

Position Overview

• Focus on home-based and community-oriented care
• Work schedule: Monday to Friday
• No weekend or overnight shifts required
• Combination of in-home visits and telehealth services
• Collaborative interdisciplinary team with strong clinical support
• Opportunity to assist patients with chronic and complex medical conditions
• Established patient base with ongoing program expansion
