Work Environment\n

This position offers a fully remote work setting, allowing you to engage with clients and collaborate with an interdisciplinary team from the comfort of your home. Key Responsibilities\n

• Provide compassionate support and guidance to patients and their families during behavioral health crises, fostering a safe space for emotional expression.

• Manage case referrals, conducting assessments to identify resource needs and care gaps, while regularly reviewing each case’s progress and determining the appropriate case closure.

• Facilitate individual recovery goals through psychoeducation, offering patients and families educational materials and direct connections to community resources.

• Evaluate social determinants of health affecting patients, addressing barriers like housing, income, food security, and transportation challenges, to ensure comprehensive care.

• Link patients and families with relevant support groups, community networks, and resources aimed at improving mental wellness.
\n
• Work collaboratively with an interdisciplinary team, including healthcare providers and specialists, to achieve cohesive treatment plans tailored to individual patient needs.

• Engage in ongoing consultations with team members to monitor patient progress and adjust care plans as necessary, ensuring the highest standards of care.

Patient Population / Client Focus\n

Your primary focus will be working with individuals and families dealing with behavioral health issues, including those with co-occurring disorders and acute mental health needs such as suicidality, and severe persistent mental illness. You will be instrumental in providing support to families of individuals in our detox facility, ensuring they receive empathetic care and are informed throughout the recovery process.

Qualifications\n

• A Master's degree in Social Work (MSW) is required, along with an active and valid license to practice in California.
\n
• Demonstrated experience (minimum 2 years) in team-based care settings, with strong collaboration skills that enhance patient care.

• Background in child and adolescent case management, particularly in outpatient and telehealth environments.
\n
• Comprehensive understanding of and experience with acute behavioral health needs, including crisis intervention strategies.
\n
• Outstanding communication skills, both verbal and written, allowing for effective interaction with clients and interdisciplinary team members.

• A solid knowledge of community resources that can be leveraged to support patient needs.
\n
• Proficient organizational skills and the ability to manage time effectively, allowing for the efficiency necessary within a busy remote setting.

• Strong problem-solving capabilities and capacity to multitask effectively.

Preferred Experience\n

• Experience working within a Detox Center or similar high-acuity settings is preferred.

• Familiarity with trauma-informed care practices and their application in behavioral health treatment.

• Additional certifications or trainings related to substance use disorder treatment and recovery support are advantageous.

Compensation and Benefits\n
