As a Physical Therapist, you will provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists. You will design and implement individualized plans of care, direct the assistance of support staff, and document evaluations, treatment goals, and progress. You will also promote Elara Carings mission and serve as a resource to the community for physical therapy services.

• Provide direct patient care and evaluate functional needs and outcomes.
• Design and implement plans of care based on assessments and patient goals.
• Document evaluations, treatment goals, and progress according to policy.
• Provide leadership to staff and ensure quality standards of care are met.
• Comply with all regulatory and organizational requirements.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved program.
• Current, unrestricted PT license in the state of practice.
• Minimum one (1) year of experience in a healthcare facility.
• Valid drivers license, dependable vehicle, and current auto insurance.
• Ability and willingness to travel within branch coverage area (approximately 50%).
• Ability to lift, push, pull, and support up to 50 pounds as needed for patient mobility.
