Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A California-licensed Pharmacist is needed to support a remote Medication Therapy Management (MTM) pilot program focused on improving patient outcomes through clinical pharmacy services. This part-time contract position offers flexible scheduling, allowing 10-20 hours per week with a guaranteed minimum of 8 paid hours. Work remotely from anywhere within California while contributing valuable MTM consultations and patient counseling as part of a Medicaid initiative.

Location: Remote (must reside in California)
Schedule:

• Flexible day shift hours for patient outreach and counseling
• Approximately 10-20 hours per week
• Minimum guaranteed 8 paid hours per week

Responsibilities:

• Conduct MTM consultations to optimize medication therapy
• Provide patient counseling and medication education to enhance adherence and understanding
• Review medication profiles and identify opportunities for therapy optimization
• Document clinical interventions and patient encounters thoroughly and accurately
• Collaborate with program stakeholders to improve patient outcomes
• Ensure compliance with all state and federal regulations

Required Qualifications:

• Active, unrestricted California Pharmacist license
• Must currently reside in California
• Previous experience conducting MTM services
• Enrollment in California Medicaid (Medi-Cal) as an ORP (Ordering, Referring, and Prescribing) Provider is required
• Strong communication skills with the ability to provide effective patient counseling
• Comfortable working independently in a remote environment with flexible hours
