This 400-bed facility focuses on improving patient outcomes by supporting medication management during the critical transition period from hospital to home. The role is ideal for pharmacists passionate about patient education and ambulatory care.

Key Responsibilities:

• Provide medication education and counseling to patients prior to hospital discharge.
• Assist patients in obtaining medication refills before leaving the hospital, ensuring continuity of care.
• Support the 'meds to beds' program designed to improve medication adherence and reduce readmissions.
• Collaborate with healthcare teams across the hospital and outpatient clinics within the health system.

Desired Experience and Qualifications:

• Licensed Pharmacist with a commitment to patient-centered care.
• Preference given to recent PGY-1 residency graduates, although non-PGY-1 pharmacists will be considered.
• Experience or interest in ambulatory care and transitions of care pharmacy practice.
• Strong communication skills and ability to counsel diverse patient populations.
