Health Navigator Job Description Template
Our company is looking for a Health Navigator to join our team.
Responsibilities:
- Develop goals and action plans together with parents of participating children to address oral health risks.
Requirements:
- Excellent problem solving and customer service skills;
- Bilingual Spanish required;
- Minimum of one year of experience in a fast-paced health care environment preferred;
- Knowledge of ICD-9 and ICD-10 preferred;
- Some college credits preferred;
- High school graduate or equivalent required;
- Completion of Accredited Community Health Worker Program preferred;
- Ability to utilize Electronic Health Records and Electronic Patient Management systems.
