Home Health Aide / HHA | PRN Job Description Template
Our company is looking for a Home Health Aide / HHA | PRN to join our team.
Responsibilities:
- Obtaining and recording accurate vital signs;
- Assist with self-administered urine tests for sugar, acetone or albumin;
- Assists only self-directed clients with the self-administration of medications;
- Caring for physical, emotional, and developmental needs of patient;
- Assisting with the use of devices for aid to daily living;
- Preparing clinical notes which includes tasks that were performed and reasons assigned tasks were not performed;
- Assist with range of motion exercises;
- Recognizing emergencies, knowledge of emergency procedures and basic home safety;
- Changes clean, non-sterile, dry dressings for simple, chronic wounds;
- Assisting with the change of ostomy appliances for stable, chronic stomas (no irrigation);
- Answer client questions about self-care techniques and give instructions that will assist family, client, and family/support in providing care;
- Assist clients with proper exercises and assist PT in the rehabilitation for client when applicable;
- Lift, turn, and weigh clients as appropriate;
- Help client maintain personal hygiene and assist with all aspects of activities of daily living;
- Bathe clients, assist with showers and partial baths as appropriate.
Requirements:
- Current CPR Certification preferred and as required by state regulations;
- Successful completion of an approved Home Health Aide training program as required by state and/or federal guidelines;
- 6 months of experience as an Home Health Aide in healthcare (home care or facilities);
- Auto liability insurance and a current and unrestricted drivers licences is required;
- Strongly preferred CNA license or as required by state regulation;
- High school graduate or GED;
- Certified Nursing Assistant (CNA) strongly preferred;
- Minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility);
- Minimum of 1 year of experience in a home health or hospice setting;
- Must be registered and listed in good standing on the Nurse Aide Registry in the state of employment.
