Home Health RN Job Description Template
Our company is looking for a Home Health RN to join our team.
Responsibilities:
- Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate;
- Assesses and coordinates patient’s discharge planning needs with members of the healthcare team;
- Notes and carries out physician and nursing orders;
- Performs other duties as assigned;
- Serves as the primary coordinator of all disciplines for well coordinated patient care;
- Responsible for participating in on-call rotation and emergency call according to agency policy;
- Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided;
- Participates in interdisciplinary patient care conferences and Agency Performance Improvement processes.
Requirements:
- Valid Drivers’s License w/ Clean DMV;
- Valid Drivers’s License w/ Clean DMV;
- Current GA State or Compact License;
- Valid Driver’s License/ Insurance;
- Ability to organize and prioritize workload independently;
- Oasis D experience or at least Oasis;
- Homecare Homebase experience;
- Must possess a valid state driver’s license;
- Graduate of an accredited School of Nursing;
- Preferred experience in Public Health, Home Care or Hospice nursing;
- Minimum Requirements: Registered Nurse, currently holding an unencumbered license to practice in the state; BSN preferred;
- Basic computer literacy with experience in Windows-based programs preferred;
- Graduate of National League for Nursing accredited school of nursing;
- BLS certified;
- 2 years of Home Health Experience.
