Our purpose and passion are to empower patients, their families, and our communities by helping individuals achieve recovery and optimal wellness of the mind, body, and spirit.administration of prescribed medications and patient education.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Monitors the effects of medications administered and documents accurately and appropriately.Reports and documents medication errors, pharmacy errors, packaging errors, medication count concerns, etc.Ensures that ongoing assessments are conducted as determined by the patient's condition using COWS & CIWAS and re-prioritizes care accordingly.Maintains neat and accurate narcotic control inventories and all documentation related to the administration of medications.Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions.Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations.Documents patient care services by charting in EMR.Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs.Participates in the onboarding of new nurses for medication administration.At end of shift communicate to incoming LPN pertinent data regarding patient activity and conducts medication count.supports cultural diversity.Knows role and responds appropriately to all codes related to emergency situations.Consistently arrives to work on time and adheres to attendance policies.QualificationsEducation/Experience:Current LPN License in State of operations.LPN Diploma & State License RequiredKnowledge of drug and alcohol treatment preferred.Working knowledge of Google ProductsPhysical RequirementsProlonged sitting at a deskMust be able to lift 15 pounds at a timeCertifications and Licenses:Current LPN License, BLS, AEDCompensation Transparency: In accordance with the Massachusetts Pay Transparency Act, the reasonable estimate of the pay range for this position $36.00 – $39.64 per hour.
