REGISTERED NURSE / RN SHIFT CARE - HOME HEALTH

We are seeking a dedicated and compassionate Registered Nurse - Float to deliver high-quality care to patients in a residential or home-care setting. This role involves implementing individualized care plans, performing clinical procedures, monitoring patient conditions, and providing supportive care with empathy and professionalism. The ideal candidate will be comfortable working independently, managing multiple cases, and adapting to different environments and shift schedules.
Key Responsibilities
Implement and follow individualized care plans tailored to each patient’s needs
Conduct home visits within assigned geographic areas
Administer medications, perform wound care, and carry out other prescribed treatments and procedures
Monitor patient conditions and report significant changes to the Clinical Manager or Client Services Manager
Accurately execute and document physician orders
Perform comprehensive patient assessments and case management when necessary
Regularly take and track vital signs
Ensure accurate medication administration and tracking
Provide catheter care, assist with feeding, bathing, hygiene, and other activities of daily living (ADLs)
Maintain clean and sanitary living conditions for patients
Track and order medical supplies as needed
Demonstrate empathy, professionalism, and reliability in patient interactions
Adapt to various shift schedules and patient needs
Drive to and from patient residences with reliable transportation
Build strong rapport with patients and family members
Work independently with confidence in providing high-quality care without direct supervision
Required Qualifications
Education
Associate’s Degree in Nursing (ADN) - Required
Bachelor of Science in Nursing (BSN) - Preferred
Experience
Must have at least 2 years of recent experience in the specialty within the past 2 years.
Preferred Qualifications (if any)
Experience with electronic medical record (EMR/EHR) systems
Background in community health or mobile/field nursing
Certifications (if any)
Active RN license in the state of practice - Required
