Identify the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. Individualize the patient's plan of care based on the identified needs. Document problems, nursing assessments, appropriate goals, care provided, care interventions, and patient and family outcomes achieved from interventions and care provided. Assess all aspects of the patient's pain and develop an individualized pain management plan. Anticipate, prevent, and treat undesirable symptoms or secondary symptoms. Participate in interdisciplinary team (IDT) meetings to establish, review, and revise the plan of care for each patient. Exchange information, review problems, and share professional support. Maintain up-to-date patient records to ensure that problems, plans, interventions, and goals are accurately stated and changes are reflected as they occur. Teach appropriate interventions, medications, effects, and side effects to patients, caregivers, and volunteers as appropriate. Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patients and families. Be responsible for the maintenance of electronic medical records, assuring accuracy, completeness, and compliance with regulations and standards. Identify and address comfort care needs. Support, instruct, and educate patients, families, and caregivers. Coordinate all patient and family services. Maintain the dignity of the dying patient. Support patients' and families' unique spiritual and cultural beliefs. Provide holistic, family-centered care to improve quality of life. Consult and collaborate with the interdisciplinary team and others involved in patient care. Provide care consistent with national standards of practice for hospice programs. Flexible scheduling required, including on-call availability. Potential to cover additional territories as needed.
