Nursing - Registered Nurse - Primary Care
InnovAge Holding Corp.
is seeking an Eligibility RN to join the In-Home Services team in Florida.
The role partners with the local Enrollment team to determine participant eligibility for PACE, schedules home visits, conducts level of care assessments, and coordinates with the primary care physician to assemble enrollment documentation. The position requires an Associate degree in nursing, active RN license, First Aid and BLS, plus 3+ years in geriatrics and 1+ year with frail or elderly
