Physical Therapist - Full Time - Home Health Care

We are seeking a dedicated and skilled Outpatient Physical Therapist to join our team at Fairfield Physical Therapy Clinic in Tampa, Florida. As a home care facility, our outpatient services are delivered directly in patients' homes. This unique setting allows physical therapists the opportunity to assess and address individual needs in a familiar environment, enhancing patient comfort and motivation. You will work closely with a diverse team of healthcare professionals to coordinate care and ensure best practices are implemented consistently. Conduct comprehensive evaluations for patients, which includes reviewing medical history, performing strength and mobility assessments, and determining functional thresholds.

Implement treatment strategies such as therapeutic exercises, manual therapy techniques, gait training, and other modalities based on current evidence and patient response.

Collaborate effectively with interdisciplinary teams, including physicians, occupational therapists, and nursing staff, to monitor patient progress and make necessary adjustments to therapy protocols.

Document all patient care activities meticulously, ensuring compliance with local and federal regulations, as well as assisting in department goal achievement.

Engage in ongoing professional development through workshops, conferences, and in-house training, also serving as a mentor to new employees and students.

Your patients will include diverse populations with a variety of conditions such as post-surgical rehabilitation, orthopedic injuries, neurological impairments, and chronic pain syndromes. Minimum requirement of a Bachelor's Degree in Physical Therapy; candidates with a Doctorate degree in Physical Therapy (DPT) are encouraged to apply.

A valid Physical Therapy license in the state of Florida is required.

Current American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification.

At least 2 years of experience as a Physical Therapist in an outpatient or home care setting is preferred.
