Key Responsibilities

• Conduct thorough evaluations including gathering patient history, performing specific assessments, and creating detailed documentation to inform the treatment plan.
• Effectively communicate evaluation findings and collaboratively set treatment goals with patients and their families, ensuring a shared understanding of care expectations.
• Implement personalized treatment protocols derived from evaluation results, continually adapting approaches based on observed patient responses and progress.
• Engage in consultations with fellow healthcare professionals as needed, documenting changes in patient conditions that may necessitate adjustments to care plans.
• Manage patient discharge planning with a focus on seamless transition to post-therapy care, ensuring timely documentation that reflects realistic patient outcomes through clear evaluation notes and progress summaries.
• Exhibit proven time management skills by maintaining productivity standards, an organized schedule, and punctuality for team meetings, while actively participating in community outreach activities.

• Take initiative in pursuing continuing education opportunities to sharpen professional skills while also serving as a clinical instructor to students in physical therapy.

Patient Population / Client Focus
