Work Environment

This role is situated within the home care setting, where you will engage directly with clients in their residences. Key Responsibilities

• Direct and implement physical therapy treatment plans aimed at preventing disabilities, alleviating pain, enhancing muscle function, and maximizing patient performance based on individual capabilities.
• Evaluate each client's functional level through rigorous diagnostic tests of muscle, nerve, and joint ability and develop tailored intervention strategies.
• Oversee and mentor physical therapy assistants, ensuring the effective execution of care plans while maintaining compliance with state laws and regulations.
• Assess home environments to recommend necessary modifications or assistive devices that foster client independence and support rehabilitation goals.
• Administer therapeutic exercises, traction, and other physical modalities, providing hands-on instruction, motivational support, and guidance during client activities.
• Collaborate actively with interdisciplinary teams, participating in case conferences and contributing to holistic care planning and evaluation.
• Document clinical records meticulously, including evaluations, care plans, and progress notes, while adhering to organizational policies related to EMR software, such as Web PT.
• Advise clients and families on the best practices for following through with their therapy regimens, ensuring they understand and can effectively use prescribed therapeutic devices.
• Continuously monitor and report changes in patient conditions, suggesting adjustments to therapy plans as needed to optimize patient care.
• Complete other responsibilities as assigned to support team objectives and enhance overall service delivery.

Patient Population / Client Focus

As a Physical Therapist at Saint Vincent Orthopedic and Sports, you will primarily work with home healthcare clients. This diverse patient population may include individuals recovering from surgery, managing chronic conditions, or rehabilitating following an injury. Your expertise will facilitate their recovery path, ensuring they receive high-quality, personalized care that addresses their unique circumstances and goals.

Qualifications

• Must have completed a Doctorate in Physical Therapy (DPT) program.
• Possess an active Physical Therapist license in the state of Florida with no history of sanctions or board proceedings.
• Exhibit exceptional proficiency in various manual therapy techniques and demonstrate success in implementing effective exercise programs tailored to client needs.
• Be familiar with electronic medical records software, especially Web PT, for documentation.
• Maintain current knowledge of evidence-based physical therapy practices and innovative treatments.

Preferred Experience

• Prior experience in home health settings is highly desirable, as is a strong understanding of medical conditions requiring physical therapy.
• A solid foundation in conducting comprehensive assessments and developing integrated care plans.
• Strong communication skills to effectively interact with patients, families, and healthcare colleagues.

Compensation and Benefits

We are pleased to offer a competitive salary range of USD 100,000 to 120,000 per year, commensurate with experience, along with a robust benefits package.
