REFERRAL COORDINATOR - AMBULATORY CARE MANAGEMENT

Under the supervision of the leadership team, manages patient referrals for program services recommended by their provider. Works with electronic platforms and documents to identify and route referrals to Ambulatory Care Management programs.

Retrieve program referrals from multiple sources (EMR, Email, Phone, Risk Reports etc. )

Tracks referral status and performs patient outreach as necessary.

Troubleshoots all program problems relating to the referral processes and escalates to appropriate leaders when necessary.

Assists leadership in ensuring program referral KPIs are accurately tracked and reported regularly.
