Full-Time Physical Therapist - Homecare Health

Motion Menders Therapist is seeking a dedicated and passionate Physical Therapist to join our team in Toms River, New Jersey. In this full-time role, you will work onsite, primarily providing home care services to patients of diverse backgrounds and ages. Your expertise will be essential in helping clients recover from injuries, manage chronic conditions, and improve their overall physical function, thereby enhancing their quality of life.

Conduct thorough Physical Therapy evaluations for a wide range of patients, including children and adults, to assess their physical capabilities and therapeutic needs.

Develop, implement, and periodically update individualized treatment plans aimed at achieving specific rehabilitation goals tailored to each patient's condition.

Prepare and maintain accurate documentation, including SOAP notes, progress summaries, and comprehensive care plans utilizing our electronic medical record (EMR) system.

Educate patients and their families on the recovery process, setting realistic expectations, and empowering them to engage in their rehabilitation.

Collaborate with multidisciplinary teams, including Occupational and Speech Therapists, to provide cohesive care for patients receiving multiple therapies.

Champion our organizational culture by embodying our core values and serving as a role model for both staff and clients.

Participate in meetings and training sessions to stay abreast of best practices and organizational updates within the healthcare field.

This role requires adaptability and a commitment to serving a diverse patient population. You will work with individuals from various age groups, races, and ethnic backgrounds, addressing a range of conditions from post-surgical rehabilitation to chronic pain management. Must hold a Master's degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy program.

Current and unrestricted licensure from the Board of Physical Therapy in New Jersey.

Willingness to conduct evaluations and treatments in-person and via telehealth, ensuring adaptability in response to patient needs.

Experience in a home care setting or familiarity with community resources applicable to rehabilitation services.
