Coordinating care with treatment providers, community resources, and support services to promote continuity of care and client progress

Exercising independent professional judgment to observe client progress and, when clinically appropriate, communicating material concerns, behavioral deviations, or safety-related observations to authorized stakeholders

Preparing and maintaining service-related documentation in the Company’s designated electronic health record system in accordance with applicable legal, regulatory, and professional standards

Communicating, as clinically appropriate, with family members, caregivers, and licensed medical or behavioral health professionals regarding service-related matters

Conducting clinical assessments within the scope of Contractor’s licensure and providing professional input to support treatment planning and recovery objectives

Providing individual, group, or family counseling and other recovery-oriented interventions consistent with Contractor’s professional credentials

Participating in meetings or information exchanges reasonably necessary to support regulatory compliance, care coordination, or service alignment, provided such participation does not establish supervision or control over the manner or means of Contractor’s performance

Master's degree in Social Work, Psychology or related field from an accredited program

Active Texas LPC or LCSW licensure

Minimum [1] years of experience working as an LPC or LCSW, preferably in a clinical setting

Flexibility to provide Telehealth as well as in-person services at our Houston 2‑3 days per calendar week (required).

Telehealth appointments requires services in all operating cities and their surrounding areas (Houston, San Antonio, San Marcos and Austin).
