A rewarding opportunity is available for a skilled Home Health Physical Therapist (PT) to deliver patient-centered therapy services in a field-based setting. This position focuses on helping patients regain functional mobility, strength, and independence through tailored plans of care in the comfort of their own homes.

Monday Friday, 8:00 AM 5:00 PM

• 13-week contract
• Perform comprehensive physical therapy evaluations, including OASIS assessments

Deliver skilled interventions such as exercise programs, gait and balance training, and fall prevention

Accurately document all care and interventions in the Home Care Home Base (HCHB) system

Participate in case conferences and adhere to regulatory home health standards

Current Physical Therapist (PT) license in the applicable state, in good standing

At least one year of recent home health physical therapy experience (field experience preferred)

Proficiency in Home Care Home Base (HCHB) highly desired; Reliable transportation, valid drivers license, and active auto insurance

Ability to work independently and effectively communicate with patients, families, and healthcare team

$2,299 - $2,432/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
