At Nascentia Health, you'll provide skilled, compassionate care to adults with chronic, acute, or complex medical needs in the comfort of their homes, helping patients maintain independence, improve quality of life, and feel supported in their own environment. We believe in listening to our nurses, supporting them, and setting them up for success every day. Home visits are scheduled Monday-Friday, 8:00 AM-4:30 PM, with no on-call requirement. What You'll Do as a Resident!
You will learn how to provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. Deliver safe, patient-centered nursing care in the home
Serve as the primary case manager, coordinating care with the interdisciplinary team
Develop and implement individualized plans of care
Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient's plan of care
Maintain compliance with all infection control and regulatory standards
Valid driver's license, auto insurance, and reliable vehicle
Strong communication and computer skills
Current New York State registered professional nurse license or may hold a Graduate Nurse License; BSN preferred (tuition reimbursement available!)
Must attain RN licensure within 90 days of hire.
Basic computer knowledge with proficiency in Microsoft Office or other Word processing, spreadsheet, and database management.
Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
Visual/hearing ability sufficient to comprehend written or verbal communication
Ability to use telephone and communicate over the phone
Frequently uses a laptop computer to enter orders and obtain product information
