Job Description
Title: Home Health Physical Therapist
Schedule: 8-hour day shift only
Requirements:

• Oregon physical therapy license required
• Minimum of 1 year skilled home health experience required
• Preferred experience with OASIS documentation
• Preferred skills: wound care, manual therapy
• Required certification: BCLS/BLS

Roles & Responsibilities:

• Conduct assessments and evaluations of patients in a home health setting
• Develop and implement individualized treatment plans
• Provide therapy interventions to improve patients' mobility and functional abilities
• Educate patients and caregivers on home exercise programs and safety measures
• Document patient progress and treatment outcomes in compliance with regulations

Compliance / Notes:

• Facility may cancel up to one shift per pay period
• Parking is available at no cost
• Reliable transportation is required for travel between patient visits
