Physical Therapist Opportunity

Pride Health is hiring a Physical Therapist to support our client's patients in Kingston, NY with coverage in Starting Kingston, and south of Kingston region for Ulster county. This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Contract type: Contract

Duration: 13-week Contract (Possibilities of extension)

Schedule: 8:30 am - 5pm Monday through Friday (5 days/wk.)

Weekly Gross Pay:

On Call/ weekend Requirement: No on call / 1 weekend per month

Reimbursement provided (from first patient to last patient)

Perform comprehensive OASIS assessments using HCHB (Homecare Homebase), including Start of Care (SOC), recertification, 30-day reassessments, and discharge evaluations in accordance with physician orders and agency protocols.

• Conduct skilled physical therapy evaluations assessing strength, mobility, balance, pain, gait, and functional limitations to develop individualized Plans of Care (POC).
• Provide skilled physical therapy interventions during routine and follow-up (revisit) appointments to maximize patient independence and functional outcomes.
• Supervise and collaborate with Physical Therapist Assistants (PTAs), ensuring delegated treatments are completed appropriately and in compliance with regulatory standards.
• Educate patients and caregivers on home exercise programs, fall prevention, safety techniques, mobility training, and disease management strategies.
• Coordinate care with physicians, interdisciplinary team members, caregivers, and support staff to ensure continuity of care and optimal patient outcomes.
• Conduct discharge visits including final assessments, patient/caregiver education, and transition planning to support safe continuation of care.

Graduate of an accredited Physical Therapy program (master's or higher).

Active, unrestricted Physical Therapist (PT) license.

Minimum 1 year of clinical experience (home health experience/ HCHB required).
