Description
Conduct thorough evaluations for patients in both inpatient and outpatient contexts while crafting personalized treatment plans
Implement physical therapy interventions that center on enhancing mobility, strength, balance, and overall function
Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to achieve optimal patient outcomes
Provide education to patients and their families about therapeutic objectives, discharge strategies, and at-home exercises
Document evaluations, interventions, and patient progress, adhering to all facility and regulatory requirements
Engage actively in care team discussions and interdisciplinary meetings to ensure seamless patient care
Education
~ Doctorate in Physical Therapy (DPT) or comparable degree from an accredited institution

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Specialized certifications (neurological, geriatric, etc.) are advantageous but not mandatory
Skills
Previous experience is highly preferred
Ability to address complex medical and mobility challenges
Excellent communication and teamwork abilities
Comfortable with digital charting and clinical documentation platforms
