Southmont is the skilled nursing community on our Washington, PA campus. Arranged as neighborhoods, our skilled nursing areas offer a wide range of care programs and services for those in need of long-term care. Expert registered nurses, licensed practical nurses, certified nursing assistants, and social workers partner with residents, their families, and their doctors to provide personalized care plans. Southmont also provides short-term rehabilitation that can include physical, occupational, and/or speech therapies.

The Licensed Practical Nurse (LPN) coordinates, plans, implements and evaluates nursing care for a designated group of residents. The role provides leadership and direction to CNAs and other caregivers to deliver and maintain quality care within the established policies, procedures, and guidelines of the PA State Nurse Practice Act. LPNs may manage the overall care of a designated group of residents with oversight by an RN Supervisor as needed. Experienced team members may be designated as the Team Leader of a specific nursing unit.

Requirements for the Licensed Practical Nurse (LPN):

Holds an LPN license in good standing from the State of PA.

Experience in Skilled Nursing and Dementia Care is preferred.

Desire to contribute to our compassionate, resident-focused culture and effectively interact with residents, resident families, and team members.
